We used to have a family friend who was a heart surgeon. He had a morbid sense of humor. I think it helped him cope with the magnitude of cracking people's chests open. Occasionally he would get a patient who was quite ancient and in extremely poor health. When asked to elaborate upon the patient's prognosis he would quip, "He died three days ago, but no one told him."
